PRESCRIPTION REIMBURSEMENT PROGRAM

The Jacksonville Transplant Alliance is offering a reimbursement on prescription medications to Mayo Clinic transplant recipients. All kidney, liver, heart, lung, and pancreas transplant recipients who have been transplanted at Mayo Clinic Jacksonville are eligible to receive this assistance. These reimbursements offer up to 2 payments of $100 each to help patients pay for their prescription medications. 

The details:

• Reimbursements are only available to solid organ post-transplant patients who have been transplanted at Mayo Clinic Jacksonville. 
• Any prescription medication is eligible for reimbursement.  Non-prescription      medications are not covered. 
• Any eligible patient may receive up to 2 separate $100 reimbursements. 
• One or more medications may be used to reach the maximum of $100 on each reimbursement.
• No personal financial information is required and there are no income limits to receive a prescription reimbursement.   

Program Update:

• Anyone who has already received a reimbursement may now request a 2nd reimbursement for up to $100.00.  A new Reimbursement Form and new receipts are required for a 2nd reimbursement.

What's required:

• A completed Prescription Reimbursement Form for each new reimbursement request.
• Receipts or copies of receipts.  Multiple receipts may be used to reach the $100 maximum benefit per reimbursement. New receipts are required for a 2nd reimbursement.
• Pharmacy statements, such as those downloaded from pharmacy websites, may also be used.
• All receipts or statements must have your name, the medication, and date on them.
• Receipts must be dated after January 1, 2023 and can include any receipts dated through the end of 2024.
• Only medications that are received through a retail pharmacy are eligible. 
• Any pharmacy, including mail-order or specialty pharmacy within the U.S. may be used. 

Please note: Once program funds are exhausted, reimbursement requests will no longer be accepted. Depending on when your reimbursement form and receipts are received, your request for reimbursement may not be fulfilled.
